Beïnvloeding sympatisch zenuwstelsel voor behandeling hart en nieren
In zijn promotieonderzoek onderzocht Ties van Brussel het mechanisme van het sympathisch zenuwstelsel (sympathetic crosstalk) op meerdere vlakken. Daarbij richtte hij zich op de effecten op het niveau van het hart en de nieren (cardiorenal axis).
Het sympathisch zenuwstelsel is dat deel van het autonoom zenuwstelsel dat stimulerend werkt op hart en longen. Het is in staat om onder meer de hartfrequentie, de knijpkracht van het hart, het samenknijpen van slagaderen, en de mate waarmee de nieren urine produceren, of juist vocht vasthouden, af te stellen.
Hart- en vaatziekten
Hiermee vormt de beïnvloeding van het sympatisch zenuwstelsel al tientallen jaren de basis voor een enorm scala aan medicamenteuze behandelingen van hart- en vaatziekten. Andersom kan ze ook ziektes uitlokken of in standhouden, zoals een te hoge bloeddruk, hartfalen, hartritmestoornissen, en nierfalen.

Part 1. Hemodynamic Effects of Sympathetic Modulation
‘In Part I of this thesis, we evaluated modulation of sympathetic activity in the cardiorenal axis and its therapeutic potential. The relevancy of this subject resurfaces on multiple levels: cardiac diseases are influenced by sympathetic activity, including heart failure, and cardiac arrhythmias. In the 1950s surgical procedures including radical lumbo-dorsal splanchnicectomy was used for the treatment of malignant hypertension. As there was a limited available pharmacological treatment options, The high risk of severe complications including orthostatic hypotension, incontinence and impotence were accepted as there was limited availability of medica! therapies.1 Nowadays, modulation of sympathetic tone on a pharmacotherapeutic level is the keystone in modern day treatment of a wide variety of cardiovascular diseases including cardiac arrhythmias and heart failure. Another sympathetic modulation is reflected in the sympathetically driven decrease in blood pressure followed by gastric bypass surgery. The findings allow for further exploration of potential novel therapies for obese patients in preventing development and treating cardiovascular disease.’
Part 2. Translational Hemodynamics in the Cardiorenal Axis
‘Part II of the thesis, we aimed to critically evaluate patient selection methods for renal angioplasty by reviewing the available RCT characteristics, letting us inspire by the diagnostic properties of intracoronary functional measurements in the cardiology field. In addition, we combined the available knowledge from pressure derived renal stenosis quantification with measurements of flow velocity. We found these measurements to be reliable and reproducible.’
